Add Chronic Diagnosis Code & Description to merge field options when creating a mail merge document
Need to have chronic diagnosis code and description in the merge field options when creating a mail merge document. Specifically when creating daily progress notes, we cannot input patient's chronic diagnosis, only their last diagnosis which is not always accurate for current treatment.

Description of Request: Require the ability to print up to 12 diagnosis codes on the patient's Charge Slips.
Requested Steps to define spec: With the impending ICD-10 requirements, patients will routinely have more than 8 diagnosis codes. The limitation of only being able to print 8 of those codes on the Charge Slips will severely affect the providers time and accuracy in completing the Charge Slip thus allowing more room for error.
Expected Result: All diagnosis from the previous visit would print on the current charge slip, allowing the provider to more easily link the procedures to the appropriate diagnosis codes for that visit.
Actual Result: Currently only the first 8 diagnosis have mail merge field capabilities.
Impact on Workflow: Without all 12 diagnosis codes printing, the providers will need refer to previous records and hand key the additional diagnosis on the current visit in order to properly link the ICD to the CPT.
