LMP, EDD and Gestation Week not an option in Field Defaults in Admin Forms. When client is attempting to add a string field for LMP, EDD, and Gestation Week, there are no options in Field Defaults.

Requested Steps to define spec:
1. Log in to EMR
2. Tools
3. Administrative Forms
4. Add String Field
5. Click on reference list icon in Default field
6. Search for LMP or EDD or Gestation Weeks
7. Click on Insert

Expected Result:
Ability to add LMP, EDD and/or Gestation Weeks as a String Field in Administrative Forms

Current Result:
Not able to add LMP, EDD and/or Gestation Weeks as a String Field in Administrative Forms

Impact on Workflow:
High, Client would have to enter a date field or text box and manually input information, also, user will need to go in and out of Form in order to grab each data. This is especially important for Ob providers who creates and uses Administrative Forms.
