I have some specialist who want to just capture if patient ever has had physical therapy, pain management procedures and so on just using a check mark. May be allow for some comments next to each selection. Allow us to add custom categories as the need changes based on specialty.

Also add Nutrition and Pediatric/Develomental areas. We have been in EMR for five years but we still use the History Recorder for all new patients.
