enable providers/staff/patients to update an exiting form . Enable signatures and dates. (ie pain management forms, immunization records, TB clearance)
We have several forms we have created in the EMR. Often times we need to have patients or staff resign the forms or update the forms. Once a form has been signed we cannot edit the patients signed form.
For example:
We have a TB consent for that the patient and MA sign and date when the patient receives the TB injection. 2 days later the MA has patient come back to document the results. We are unable to document the results on the original form that was used. Another example is the pain management agreement that the physician documents on and the patient reviews , initials dates, and signs. This agreement is reviewed periodically, however, we are unable to make additions or have the patient document on the original agreement.
It would be much more efficient if we could use a few forms for multiple related documentations
